4). NOI application checklist

5). NPDES fee $250.00 made to Clean Water Fund

6). Location map

7). PNDI results (less than 1 year old)

8). Act 14 notices specifying Act 67 & 68 coordination and proof of receipts

9). 1 set Erosion & Sediment Pollution Control plan & narrative

10). 1 set PCSM plan and narrative

Non-permitted Erosion & Sediment Control plans:

1). District application and fees

2). 1 complete set of Erosion and Sediment Pollution Control plans and narrative.

NPDES administrative resubmission:

1). $250.00 Clean Water Fund fee

2). Items requested in administrative incompleteness letter.

Technical deficiency resubmission: 

1). *25% of original fee for first resubmissions, 50% for second, full fee for each after.

2). Revised District application

3). 1 set of plans and/or narratives

General NPDES Major Modifications & Additional Phases 

1). Updated complete Notice of Intent

2). Notice of Intent completed checklist

3). Current PNDI results 

4). Act 14 notices specifying Act 67 & 68 coordination and proof of receipts
