3rd & 4th Registration Forms
The first three registration forms
are mandatory for Envirothon participation
- The first registration form you must submit to the Conservation
District is the SCHOOL
REGISTRATION FORM. This is your "intent" to participate in the
2009 York County Envirothon. This form is due by Friday, December 5, 2008.
Please fill in the form completely and be sure to put a full mailing address
for each coach on the back of the form.
- The second registration form you must complete and submit is the TEAM
REGISTRATION FORM. This is the most important registration form! It must
be submitted by Friday, March 6, 2009. This is your form where you enter
your official team name and order your team & coach T-shirts. Please use
one form per team!
- Your third registration form is your STUDENT
REGISTRATION FORM. You will fill out this form with your team member
names and turn it in on the morning of the competition. Please use one form
per team.
The following forms are for voluntary
3rd & 4th grade fun contests. They are due with your Team
Registration Forms on March 6, 2009, if you and your students choose to enter.

MOST CREATIVE
NAME contest entry form. This form must be filled out by the students.
One form per team.

BEST TEAM
EVER contest entry form. This form must be filled out by the coach -
please combine your teams as your "best team ever."

COACH
OF THE YEAR contest entry form. This form must be filled out by the
students - please have both your teams work on this together.
