Additional Clearance Requirements *

I have not lived outside the Commonwealth of Pennsylvania in the last 10 years and hereby swear and affirm in writing that I am not disqualified from service based upon a conviction of an offense under §6344 of the Child Protective Services Law.
I have lived outside the Commonwealth of Pennsylvania in the last 10 years and thereby must submit a fingerprint based federal criminal history (FBI) through the Pennsylvania State Police or its authorized agent.

I understand that I might be exposed to sensitive information during my volunteer time and will not copy, reproduce, or otherwise share or distribute that information without permission from the Conservation District. I understand that while volunteering, I am a representative of the Conservation District and that if my behavior does not serve as a positive representation, I may be asked to leave.
